Disability

The disability benefits consist of a Short-term Disability (STD) Plan, Long-term Disability (LTD) Plan, and an Extended Illness Bank (EIB) for certain eligible team members. The plans work together to replace a portion of your income when you are on an approved absence from work for your own serious health condition.

Plan Benefit Amounts And Guidelines
Extended Illness Bank (EIB)
  • Full-time and part-time members receive a benefit for income replacement based on accumulated hours prior to 12/14/2014
  • Income replacement at 100%
Short-term Disability (STD)
  • Provides coverage after a seven-day elimination period
  • Maximum duration is 90 days per event
  • $2,000 maximum weekly benefit
  • Team members with less than 5 years of service may purchase a supplemental buy-up plan to increase income replacement up to 80%
  • Team members enrolled in the STD buyup plan have a $2,500 maximum weekly benefit
Long-term Disability (LTD)
  • Provides coverage after a 90-day elimination period
  • Replaces 50% of your pre-disability pay ($12,000 monthly maximum)
  • Pre-existing conditions apply 1

  • Replaces 50% of your pre-disability pay ($1,000 per week maximum)
Long-term Disability (LTD)
  • Provides coverage after a 90-day elimination period

Physician Individual Disability Insurance (IDI)
  • Bi-annual enrollment opportunity for newly eligible providers
  • Covers more of your compensation, above LTD plan benefit maximums
  • Discounted premiums
  • Policy belongs to you and is portable
  • Premiums are rate-locked

Your effective date of coverage is the first of the month following your date of hire or transferring into a benefit eligible classification.

Example: When Short-term Disability (STD) Pay Begins

  • If date of disability is January 15, 2024.
  • Waiting period would be January 15-21 (PTO can be used during the seven-day waiting period).
  • STD pay (income replacement) begins January 22, 2024.
  • STD pay ends April 14, 2024 (90th day of disability)
